You fear falling into serious sin, and falling out of God's grace. You do not want that to happen to you. You hope to receive the Divine Mercy of God, and be with Him in His Perpetual Glory.
Truly repent of all your sins.
Pray the Act of Contrition: "O My God, I am heartily sorry for having offended Thee, and I detest all my sins, because of Thy Just Punishment: The loss of Heaven and the pains of Hell. But most of all, because they offend Thee, My God Who are so good and Deserving of all my love. I firmly resolve, with the Help of Thy Holy Grace, to sin no more, and to avoid the most near occasion of sin. Amen."
Pray the Holy Rosary daily, or the Chaplet of Divine Mercy, daily.
Go to Confession regularly. Christ gave His priests the power to forgive sins, according to the Gospel of John, chapter 20.
Contemplate Our Lord's Passion. Remember, He paid the price for all your sins.
